Cumulative Student File

Report Purpose

The Cumulative Student File report produces a printable student record that consolidates historical demographic, enrollment, academic, attendance, assessment, program participation, behavior, and contact information into a single report suitable for inclusion in a student's cumulative file.

Rather than displaying all information in a single table, the report is organized into multiple sections. Each section represents a different category of student information and is formatted independently when exported to PDF.

The report supports:

  • Student cumulative file maintenance

  • Student record transfers

  • Administrative record reviews

  • Case management

  • Historical student record documentation

  • Parent or guardian meetings

  • Student support planning

Report Output Format

Grain: One report per student

Each report contains multiple sections. Every section is rendered as an independent table.

Sections include:

  • Enrollment History

  • Attendance History

  • Program Participation

  • Marks and Credits

  • Assessments

  • Behavior History

Section Organization

The report is divided into the following sections.

Student Information

Displays current demographic information including:

  • District

  • Student name

  • Student ID

  • State Student ID (SSID)

  • Date of birth

  • Gender

  • Federal Race Ethnicity

  • Mailing address

  • Current grade

  • Current school

  • Student email

  • Age

  • Primary language

  • Home language

Contacts

Lists student contacts and emergency contacts.

Includes:

  • Contact name

  • Relationship

  • Mailing address

  • Home phone

  • Cell phone

  • Work phone

  • Email

  • Preferred language

  • Emergency contact indicator

  • Custodial parent indicator

Ordered by Contact Priority Order

Enrollment History

Displays historical enrollment records for the selected school years.

Includes:

  • School year

  • School

  • Grade

  • Full-Time Equivalent (FTE)

  • Enrollment start date

  • Enrollment end date

Only completed enrollment records are included.

Attendance History

Displays annual attendance summary information.

Includes:

  • School year

  • School

  • Grade level

  • Attendance rate

  • Days attended

  • Days absent

  • Chronic absenteeism indicator

A student is considered chronically absent if their attendance rate is less than 90%.

Program Participation

Lists student participation in selected educational programs.

Depending on the selected parameters, programs may include:

  • English Learner (EL)

  • Special Education (SPED)

  • Section 504

  • Title I

  • Talented & Gifted (TAG)

  • Migrant Education

  • Indian Education

  • Homeless

  • Pregnant & Parenting

  • Free and Reduced Lunch

For each membership, the report includes:

  • School year

  • School

  • Grade

  • Program

  • Program category

  • Membership start date

  • Membership end date

Marks and Credits

Displays the student's course marks and earned credits.

Includes:

  • School year

  • School

  • Grade

  • Marking period

  • Course

  • Final mark

  • Earned credit

Only final marks are included.

Assessments

Displays assessment results for the selected assessment vendors.

Depending on the selected parameters, assessments may include:

  • ODE State Assessments (OSAT/ELPA)

  • i-Ready

  • DESSA

  • STAMP

  • Other supported assessment vendors

Each assessment includes:

  • Assessment name

  • Assessment date

  • Administration window (when available)

  • Student score

  • Percentile score

Only composite (total) assessment results are included where applicable.

Behavior History

Displays reported discipline incidents.

Includes:

  • School year

  • School

  • Grade

  • Offense type

  • Incident location

  • Action taken

  • Incident date

Only reported discipline incidents are included.

Parameters

Schools

Select one or more schools.

The report includes students associated with the selected schools.

School Years

Select one or more school years to include in the cumulative record.

Historical information is filtered to the selected years.

Program Types

Select one or more program categories to include in the Program Participation section.

Examples include:

  • English Learner

  • Section 504

  • Special Education

  • Title I

  • Homeless

  • Migrant Education

Assessment Vendors

Select one or more assessment providers to include in the Assessments section.

Examples include:

  • ODE (OSAT)

  • ODE (ELPA)

  • Curriculum Associates (i-Ready)

  • Avant (STAMP)

  • Aperture (DESSA)

Show SSIS

If no, the SSID value will be displayed as “********”.

Student Population

The report includes students who:

  • Match the selected school filter

  • Meet applicable security permissions

  • Have at least one qualifying record in the selected data domains

Historical sections only include records from the selected school years.

How to Use This Report

1. Select Parameters

Choose:

  • Schools

  • School Years

  • Program Types

  • Assessment Vendors

2. Run Report

Generate the report for one or more students.

3. Export to PDF

Export the report to PDF for printing.

4. File in Student Cumulative Record

Print the report and include it in the student's cumulative folder as needed.

Notes

  • One PDF report is generated for each student.

  • Each section of the report is presented as a separate table.

  • Enrollment, attendance, assessments, behavior, marks, and program participation are historical and filtered by the selected school years.

  • All information displayed is subject to the user's security permissions at runtime.
